Oral Glucose Tolerance Test (OGTT) is a standard diagnostic test used to detect disorders of carbohydrate metabolism, such as impaired glucose tolerance and diabetes mellitus. The test evaluates the body’s ability to metabolize glucose after an oral load of a concentrated glucose solution.
Indications
- diagnosis of carbohydrate metabolism disorders (prediabetes, diabetes mellitus)
- insulin resistance
- metabolic disorders associated with obesity
- assessment of glucose metabolism in the presence of risk factors
For pregnant women:
- screening for gestational diabetes
In our practice, as well as in some clinical protocols, a stepwise approach may be used: initially, fasting venous blood glucose is measured. If the result is abnormal or risk factors are present, the oral glucose tolerance test is recommended after a few days.
In pregnant women, the test is performed between 24–28 weeks of gestation, when physiological insulin resistance is most pronounced and the risk of gestational diabetes is highest.
Procedure
The patient arrives in the morning on an empty stomach. A venous blood sample is taken to determine baseline glucose levels. The patient then drinks a solution containing 75 g of glucose. Follow-up blood samples are collected at 60 and 120 minutes after the load to assess glucose dynamics.

Preparation:
- fasting for 8–12 hours before the test (water is allowed)
- 3 days before the test, a normal diet with adequate carbohydrate intake is recommended
- avoid alcohol consumption 24 hours before the test
- avoid intense physical activity the day before the test
