Fecal occult blood test, FOBT

The fecal occult blood test (FOBT) is a laboratory analysis used to detect microscopic traces of blood in stool that are not visible to the naked eye. It is widely used for early diagnosis of gastrointestinal diseases, including inflammatory conditions, ulcers, polyps, and neoplasms.

The presence of occult blood may indicate damage to the lining of various parts of the digestive tract. This test is commonly used as a screening tool to detect conditions at an early stage.

Indications

  • Screening for gastrointestinal diseases
  • Suspicion of occult gastrointestinal bleeding
  • Unexplained abdominal pain
  • Anemia (especially iron-deficiency anemia)
  • Bowel habit changes (constipation, diarrhea, alternating patterns)
  • Suspicion of intestinal polyps or neoplasms
  • Preventive screening (especially after age 40–50)
  • Monitoring of gastrointestinal conditions

Procedure

  • The patient collects a stool sample in a sterile container
  • Collection is performed at home
  • A small amount of sample is delivered to the laboratory
  • The procedure is non-invasive and painless

Method of testing

The test is performed using a chemical method (benzidine test), based on the peroxidase activity of hemoglobin. In the presence of blood, a color reaction occurs, indicating the presence of occult blood in the stool.

Preparation:

  • Avoid red meat, liver, and fish for 2–3 days before the test
  • Avoid certain vegetables and foods that may affect the result (beetroot, broccoli, cauliflower, etc. )
  • Avoid iron supplements, NSAIDs, and other medications that may irritate the gastrointestinal mucosa (as advised by a doctor)
  • Do not perform the test during menstruation
  • Postpone the test in case of bleeding hemorrhoids or anal fissures (if possible)
  • Use a clean, dry container
  • Avoid contamination of the sample with urine or water
